Agape Behavioral Health is seeking a Telehealth Therapist to deliver group, individual, and family therapy for patients in South Florida. The role conducts psychosocial assessments and collaborates with the treatment team to formulate master plans and coordinate discharge planning.

Remote position requires a valid Florida license (LMHC, LCSW, LMFT) and a commitment to comprehensive psychosocial care within a multidisciplinary team.

Qualificações

• Master's degree required.
• Licensure as LCSW, LMFT, or LMHC in Florida is required.
• Minimum 1 year experience in substance abuse or mental health treatment preferred.

Responsabilidades

• Review assessments from referring facilities and identify issues needing attention.
• Coordinate with referral sources and maintain ongoing contact with involved parties.
• Gather diagnostic information via psychosocial assessments and prepare integrated summaries.
• Develop comprehensive treatment plans with clear goals and measurable objectives.
