Job summary

Bakersfield Post Acute is seeking a compassionate Speech-Language Pathologist for telehealth per diem engagement in California. The role focuses on evaluating and treating residents with speech, language, cognitive-communication, and swallowing disorders while coordinating with physicians, nursing staff, and families.

The ideal candidate has SNF/post-acute rehab experience, strong clinical judgment, and proficiency with telehealth platforms and electronic medical records.

Qualifications

• Master’s or Doctoral degree in Speech-Language Pathology from an accredited program.
• Current California SLP license in good standing.
• ASHA CCC preferred.
• Experience in skilled nursing, long-term care, geriatric, or post-acute rehabilitation preferred.
• Telehealth experience preferred.
• Proficiency with Net Health and PointClickCare.
• Ability to use videoconferencing technology for remote treatment.
• CPR certification if required by licensure or facility policy.
• Supervise CFs or students when applicable, per state requirements.

Responsibilities

• Conduct comprehensive evaluations of speech, language, cognitive-communication, voice, and swallowing needs.
• Develop, implement, and update individualized plans of care with measurable goals.
• Provide medically necessary treatment via telehealth platform.
• Monitor progress and modify treatment plans as needed.
• Educate residents, families, and caregivers on treatment goals and strategies.
• Communicate with physicians, nursing staff, and rehab team about progress and discharge planning.
• Participate remotely in care conferences and rehabilitation meetings.
• Complete evaluations, progress notes, and discharge summaries in a timely, compliant manner.
• Supervise Speech-Language Pathology CFs or students where applicable.
• Maintain resident privacy and confidentiality during telehealth.
