Fuze Health is seeking a Registered Nurse to support our telehealth care team by providing remote outreach to Medicare members and ensuring compliance with Stars measures. The role focuses on safe discharge follow-up and contributing to additional HEDIS measures while delivering compassionate virtual care.

Shifts are Monday to Friday, 12:30 PM to 9:00 PM EST with one Saturday per month. This full-time position offers a comprehensive benefits package and opportunities to impact patient outcomes

Formación

• BSN degree required.
• Active, non-restricted nursing license with primary state licensure.
• 2+ years of RN experience.
• 1-2 years in remote healthcare/telehealth.
• Strong computer skills and EMR familiarity.
• Strong customer service and interpersonal skills.
• Ability to work independently and in a team.
• Visa sponsorship not available at this time.

Responsabilidades

• Provide remote telehealth patient care and support.
• Conduct virtual patient assessments including medical histories and symptoms.
• Document patient conditions, histories, and care interactions.
• Educate patients and families on conditions, medication adherence, and self-care.
• Collaborate with other healthcare providers to implement care plans.
• Maintain regulatory compliance and privacy standards.
• Stay updated on telehealth practices and medical advancements.
