Job summary

Urrly is hiring a Telehealth Nurse Practitioner to conduct Comprehensive Health Assessments for Medicare/Medicaid members from a fully remote setting. The role requires licensure in at least two states, strong documentation, and familiarity with HEDIS and risk adjustment workflows.

Qualifications

• Active NP license and multi-state eligibility.
• Experience in Family or Internal Medicine preferred.
• Familiarity with Medicare/Medicaid programs and risk adjustment workflows.

Responsibilities

• Conduct Comprehensive Health Assessments via telehealth visits.
• Review medical history, medications, and preventive care needs.
• Document HCC risk adjustments accurately during visits.
• Identify and close HEDIS care gaps and document using ICD-10/CPT II codes.
• Deliver clear care plans and follow-up guidance to members.
• Maintain timely, structured documentation to support value-based care workflows.
