Job summary

Baptist Health is seeking a remote System Clinical Pharmacy Specialist to lead OPAT/COpAT services across its health system. Qualifications

• Bachelor's degree in pharmacy or Doctorate of Pharmacy required.
• Kentucky and Indiana license eligibility; preceptor designation required in Kentucky.
• Board certification in infectious diseases pharmacotherapy preferred (BCIDP) or eligible within 12 months.

Responsibilities

• Develop, implement, and maintain OPAT/COpAT services across the health system.
• Review antimicrobial regimens, monitoring plans, and culture data for outpatient therapy.
• Coordinate transitions of care and optimize antimicrobial stewardship dashboards and reporting.
• Educate pharmacists, providers, and care teams on OPAT/COpAT best practices.
