Job summary

Holon Health is seeking an experienced Nurse Practitioner licensed in Wisconsin to support individuals with SUD through telehealth-based, integrated care. Work with Care Navigators, Recovery Coaches, and other partners to address mental, emotional, and physical needs.

The role offers remote flexibility and can be full-time (40 hrs/week) or part-time (20 hrs/week). Deliver assessments, medication management, and care coordination to advance recovery and remove barriers to care.

Qualifications

• Active and unrestricted NP license in Wisconsin.
• Experience in behavioral health, addiction medicine, or psychiatry.
• Board certification in specialty preferred.
• Strong documentation skills and proficiency in EHR.

Responsibilities

• Conduct health evaluations to assess substance use and related conditions.
• Diagnose conditions and create individualized treatment plans.
• Prescribe and manage medications via telehealth and in-person.
• Collaborate with care teams to deliver integrated, whole-person care.
• Educate clients and families on plans, medications, and recovery support.
• Maintain accurate documentation in line with standards.
• Participate in team meetings and professional development.
