Holon Health is seeking an experienced Nurse Practitioner licensed in Missouri to support individuals with SUD through collaborative, integrated care. The role offers remote flexibility with full-time or part-time options, focusing on telehealth assessments, medication management, and care coordination to support recovery journeys.

You will work with Care Navigators, Recovery Coaches, Case Managers, judicial partners, and community organizations to address mental, emotional, and physical needs

Competenze

• Licensed NP with Missouri license and active DEA license.
• Experience in behavioral health, addiction medicine, or psychiatry.
• Strong documentation and EHR skills.
• Ability to deliver telehealth assessments and care coordination.

Mansioni

• Conduct health evaluations for SUD and co-occurring conditions.
• Diagnose and create individualized treatment plans.
• Prescribe and manage medications; monitor for side effects.
• Collaborate with care team and external providers for coordinated care.
• Act as clinical consultant on complex cases.
• Educate clients and families about treatment plans and recovery support.
• Maintain compliant documentation.
• Participate in team meetings and ongoing training.
