Job summary

Holon Health in Des Moines, IA is seeking an experienced Nurse Practitioner licensed in Iowa to support individuals with SUD through collaborative, integrated care. In this role, you'll work with Care Navigators and Recovery Coaches to deliver comprehensive, whole-person care via telehealth and in-person as needed.

The position offers remote flexibility and can be full-time or part-time. You will perform telehealth-based assessments, medication management, and care coordination to support

Qualifications

• Active and unrestricted NP license in Iowa.
• Active DEA license (and CDS where applicable).
• Board certification in a related specialty preferred.
• Several years of experience in behavioral health, addiction medicine, or psychiatry.

Responsibilities

• Conduct health evaluations to assess substance use and co-occurring conditions.
• Diagnose health conditions and create individualized treatment plans.
• Prescribe and manage medications, monitor side effects, adjust as needed.
• Collaborate with the Holon care team and external providers for coordinated care.
• Serve as a clinical consultant on complex or high-need cases.
• Educate clients and families on plans, medications, and recovery strategies.
• Maintain accurate documentation in line with standards.
• Participate in team meetings and ongoing training.
