Position: Remote Staff Physician - FLORIDA

Monogram Health is looking for a skilled Physician eager to make a difference in patients' lives. The Staff Physician at Monogram Health is a key member of the Florida clinical team, which includes over 80 APPs, RNs, SWs, and pharmacists. The patients we serve often struggle with multiple serious diseases. Our Physicians help improve their quality of life at home and slow the progression of kidney disease, enabling positive health outcomes.

Your Impact

Using your skills, you will support the quality and effectiveness of Monogram's care model, delivering personalized, compassionate medical care to individuals with chronic kidney disease. Responsibilities include reviewing care plans, chart audits, peer discussions, provider training, and coaching. You may also care for patients via telemedicine. Our goal is to prioritize patient well-being over systemic processes and incentives, putting patients and their quality of life first.

Roles And Responsibilities
  • Support Florida Market Physician Executives with care plans, audits, training, and other duties
  • Participate in care team meetings
  • Assess patient health status and develop treatment plans
  • Implement and review treatment plans and clinical documentation
  • Provide clinical training and coaching to team members
  • Engage in peer discussions and communicate with community providers via EMR
  • Conduct assessments, including annual wellness exams, primarily virtually
  • Prescribe medications and order tests
  • Document all clinical activities thoroughly and timely
  • Coordinate care with other providers and services
  • Maintain quality standards and adhere to regulations such as HIPAA and OSHA
  • Attend periodic training/meetings outside Florida as needed
  • Aim for improved patient outcomes and closing care gaps through education
  • Perform other duties as assigned
Position Requirements
  • Basic Life Support (BLS) certification (company supports certification process)
  • Active, unrestricted MD license
  • Board certification appropriate to licensure
  • Unrestricted DEA certificate
  • Ability to work independently and autonomously
  • Valid driver's license, transportation, and insurance
  • Proficiency with medical instruments, equipment, and data management systems
  • Effective communication skills with diverse stakeholders
  • Knowledge of federal and state regulations, including HIPAA and OSHA
  • Minimum 2 years of direct patient care experience
  • Experience with Managed Care/IPA/Health Plans
  • Experience conducting annual wellness or similar comprehensive visits virtually or in-home
