Preventel Health in New York is seeking a remote CCM/RPM care manager who speaks Spanish to deliver health and wellness calls to assigned patients. The role focuses on chronic care management and remote patient monitoring by telephonically supporting patients.

Responsibilities include review and assessment, coaching, care plan development, outcome monitoring, and enrollment facilitation. Must reside in NY/CT/TX/NC/OR and possess FMG (or related) certification and relevant experience.

Competenze

• CCM/RPM experience is required.
• Excellent customer service skills are essential.
• Proficient with Microsoft Office.
• Must have computer with internet access.

Mansioni

• Review and assess patients based on disease states.
• Remotely provide patient coaching and care to improve outcomes.
• Develop a care plan with patient, family/caregiver, and provider.
• Monitor outcomes, adherence, and progress; adjust as needed.
