Job summary

Preventel Health in New York is seeking a remote CCM/RPM care manager who speaks Spanish to support chronically ill patients by phone. You will collaborate with families, providers, and community resources to deliver targeted coaching and sustain continuity of care.

Develop care plans, monitor outcomes, and guide patients through CCM and RPM enrollment. Strong customer service, MS Office skills, and reliable internet access are essential for success in this remote role.

Qualifications

• Pharmacy intern, Medical Assistant (MA) Foreign Medical Graduate Certification (FMG).
• CCM/RPM experience.
• Must be in NY, CT, TX, NC or OR.
• Excellent customer service skills.
• Knowledgeable using Microsoft Office.
• Computer with internet.
• Plus excellent incentive model.

Responsibilities

• Review and assess patients based on disease states.
• Remotely providing patient coaching and care to improve patient outcomes.
• Develop a care plan with the patient, family/caregiver(s) and provider.
• Monitor patient outcomes, adherence to care plans, evaluate effectiveness, monitor patient progress in a timely manner and facilitate changes as needed.
• Coaching and educating patients on improving their Chronic Conditions, preventive care and physician directives.
• Telephone outreach to Spanish and English speaking patients.
• Facilitate CCM and RPM program enrollment; Concise and accurate documentation.
• Cultivate and support the primary care providers with timely communication, inquiry follow-up, and integration of information into the care plan regarding transitions-in-care and referral.
