Job summary

Healthmap Solutions Puerto Rico is seeking a Social Worker to provide outreach and linkage to community resources for members, in collaboration with Care Navigators.

The role involves conducting assessments, providing supportive counseling, and coordinating with outside agencies to ensure continuity of care. MSW is required; LCSW preferred, with at least 3 years in healthcare or human services.

Qualifications

• MSW required.
• Active LCSW license preferred.
• 3 years experience in healthcare or human services setting.
• Experience in a managed care setting.
• Experience with community resources preferred.

Responsibilities

• Outreach to members and identify psychosocial needs.
• Assist in linking to community resources and referrals.
• Document in Care Management Systems.
• Provide in-home evaluations when needed.
• Collaborate with outside agencies to ensure continuity of care.
• Participate in team meetings and trainings.
