• Conducts comprehensive assessments of patients' social, emotional, environmental, and financial needs.
• Plans course of treatment or referrals to appropriate professionals to ensure proper care.
• Collaborates with the multidisciplinary team to coordinate care and monitor progress.
• Provides education to physicians, nurses and other staff on community resources and psychosocial care.
• Documents interactions and outcomes across data management systems.
