Texas Children's Hospital is seeking a Medical Social Worker to support patients and families through psychosocial adaptations related to diagnosis and mental/behavioral health. You will help determine eligibility for social programs and coordinate funding resources within the care team to support discharge planning.

You will conduct assessments, plan and implement Care Plans, and collaborate with interdisciplinary colleagues to ensure timely access to services and community resources.

Competenze

• Master’s degree in social work required.
• LMSW or LCSW license from the Texas Behavioral Health Executive Council required.

• Conduct psychosocial assessments evaluating patient/family risk factors.
• Collaborate with interdisciplinary team to implement and follow-up on the Psychosocial Plan of Care.
• Identify department needs and participate in problem-solving through staff meetings and committees.
• Participate in program development for improved quality of care and service delivery.
• Serve as a resource regarding medical/legal reporting guidelines to the team.
• Provide evidence-based, client-centered therapy to patients and families.
