Overview

This position is a remote full-time/day role (IUHMG-Metro).

The Virtual Integrated Behavioral Health team provides innovative services through a virtual model, collaborating with outpatient healthcare providers. Responsibilities include supporting patients' mental and physical health, coordinating with medical providers, screening and assessing for mental health and substance use disorders, facilitating engagement and follow-up care, tracking treatment response, and supporting psychotropic medication management. They employ brief, evidence-based behavioral interventions and consult with psychiatric specialists for treatment updates. Team members also develop relapse prevention plans for patients who have achieved their treatment goals.

Key responsibilities include:

  • Effectively communicating and collaborating with all members of the healthcare team to ensure safe, timely patient services, and providing necessary resources and education to patients and families.
  • Interpreting and communicating complex patient/family needs and role modeling caring practices to other caregivers.
  • Assessing psychosocial risk factors through evaluation of prior functioning, support systems, reactions to illness, and coping abilities.
  • Counseling patients and families on emotional, social, and financial impacts of illness/disability and mobilizing community resources.
  • Providing mental health assessments upon physician referral.
  • Facilitating complex discharge planning and serving as a liaison with protective teams and in guardianship, foster care, and mental health placements.
  • Providing specialized mental health services, including crisis intervention and addiction assessments, and recommending psychiatric or community resources.
  • Supporting patients in crisis, utilizing de-escalation techniques, and intervening in dangerous situations.

Minimum requirements:

  • Master's Degree in Social Work.
  • De-escalation certification within 6 months.
  • Permanent LSW licensure (LCSW preferred).
  • 3-5 years of experience in mental health, child and family services, adult protective services, VA, or substance abuse.
  • Experience with diverse populations and knowledge of legal mandates.
  • Proficiency in Microsoft Excel and Word.
