Job summary

Green Key Resources is seeking a Social Work Case Manager to join a Managed Long-Term Care (MLTC) team in Brooklyn, NY. You will coordinate services, review person-centered care plans, and conduct ongoing member outreach to ensure appropriate services are provided.

The hybrid schedule offers 3 in-office days and 2 remote days, Monday–Friday, 9:00 AM–5:00 PM, with a focus on MLTC/Medicare managed care experience.

Qualifications

• MSW, LMSW, or LCSW required.
• Minimum 2 years of case management experience.
• MLTC/Medicare managed care experience preferred.

Responsibilities

• Coordinate services and review person-centered care plans.
• Conduct ongoing member outreach to ensure engagement.
• Ensure members receive appropriate covered and non-covered services.
