Job summary

Central Ohio Area Agency on Aging is seeking a Service Coordination Assistant to support coordination of community-based services and monitor delivery to individuals. You will schedule, refer providers, and collaborate with teams to ensure services are implemented as planned.

You will educate members about options, assist with transportation, document all interactions, and generate MARs while maintaining confidentiality and compliance with program guidelines.

Qualifications

• High school diploma or equivalent.
• 1 year of experience in social services, case management support, or healthcare coordination.
• Strong communication and organizational skills; ability to multitask.

Responsibilities

• Schedule appointments and coordinate/provide monitoring of services as planned.
• Collaborate with coordinators and teams to address overall service needs.
• Educate individuals on options, rights to appeal, and self‑directed services.
• Document correspondence and generate Monthly Activity Reports; maintain confidentiality.
• Provide transportation coordination and support onboarding and training.
