Job summary

Parallon, part of HCA Healthcare, seeks a Core Measures Data Abstractor II focused on Sepsis. This work-from-home role abstracts data for complex core measures across facilities within the network.

You will review medical records, perform timely data abstraction into systems (COMET, TheraDoc, NHSN, etc.), and ensure compliance with CMS, TJC, NHSN standards; other duties include follow-ups, education, and maintaining clinical knowledge.

Qualifications

• 2+ years of Health Information Management experience and/or registry abstraction.
• Certification in coding or nursing preferred.
• RHIT, RHIA, CCS certifications preferred.
• LVN or RN preferred.
• Undergraduate healthcare degree required; 5+ years may substitute.

Responsibilities

• Completes abstraction for assigned facilities into required systems.
• Reviews medical records to abstract data per CMS, TJC, NHSN standards.
• Performs timely abstraction to maintain compliance.
• Conducts edit checks and applies necessary changes promptly.
• Follows standards to abstract all reportable cases and assist with follow-up.
• Maintains clinical knowledge and communicates with management to achieve measure compliance.
