Job summary

Sagility is hiring a Senior Care Specialist-Clinical Operations to manage care plans for members remotely. You will assess needs, coordinate services, and support transitions of care for Medicare/Medicaid populations while collaborating with a multidisciplinary team.

Requires BSN/MSN and 3+ years of clinical nursing, with strong communication, problem solving, and proficiency in Excel and EHRs. Work-from-home role in the United States.

Qualifications

• Minimum of 3 years of clinical nursing experience.
• Active nursing license (non-compact states) and reside in the state of the compact license.
• Experience with care management and chronic conditions.
• Proficiency with MS Excel and office software.
• Strong telephonic and virtual communication skills.

Responsibilities

• Telephonically assess Medicare, Medicaid, and/or Group Account members and create actionable care plans.
• Use nursing judgment to assess and coordinate care for acute situations.
• Discuss transitions of care to assist with safe discharge to home and coordinate care services.
• Guide members and families toward resources for care and wellbeing.
• Assess health issues in collaboration with a multidisciplinary team and provide timely interventions.
