Job summary

Sagility is seeking an experienced Registered Nurse to join as a Senior Care Specialist in a work-from-home capacity. You will assess members by phone, develop individualized care plans, and coordinate services to improve health outcomes for those with chronic conditions.

The role uses a Whole Person Care approach, collaborating with care teams to identify risks, address barriers to care, and support transitions in care. Seasonal opportunity through June 2027 with possible bonuses and incentives.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on member needs and risks.
• Establish health coaching relationships and provide education related to chronic condition management.
