Job summary

LifeLong Medical Care is seeking a Registered Nurse II - Triage to deliver patient care via telephone triage and our electronic health record. This Hybrid/Remote role requires CA RN licensure and residency in California.

The nurse will participate in coordinated care, relay patient information to clinicians, and support a diverse patient population within a collaborative team. The position supports a strong primary care model, uses EPIC, and emphasizes compassionate, patient-centered care.

Qualifications

• Valid California RN license (unencumbered).
• BLS certification.
• 1 year as LifeLong Medical Nurse or 2 years RN experience.

Responsibilities

• Make quick decisions, multitask and manage stress while communicating clearly.
• Coordinate care for patients via telephone and MyChart messages.
• Follow up with patients and ensure communication with the primary care team.
