Qualifications

• RN license active and unrestricted in the state of residence.
• 4+ years of case management experience.
• 3+ years of clinical experience.
• 1+ year of leadership or supervisory experience.

Responsibilities

• Lead clinical performance improvement projects and manage multiple complex deliverables to implementation.
• Drive Stars and quality improvements and focus on utilization and cost.
• Establish trusting relationships with provider partners for open communication.
• Interpret data and educate teams on PC performance improvement.
• Collaborate with Engagement Managers, Local Market Team, Data Team and others.
• Coordinate clinically focused meetings to identify opportunities and priorities.
