Job summary

The Medical University of South Carolina (MUSC) seeks a Nurse Care Coordinator to support a patient-centered medical home program. This remote role involves travel for rounding and clinic work as needed, with 40 hours per week and hourly pay.

SC residency is required to match the program guidelines. The incumbent collaborates with physicians, staff, and other health professionals to improve care delivery, track quality measures, and facilitate effective team communication for an assigned patient

Qualifications

• Must hold a Bachelor's in Nursing and be a licensed RN in South Carolina.
• Minimum three years nursing experience with care coordination or CCM.
• Certification in a related specialty by ANA is preferred.

Responsibilities

• Identify patients qualifying for care coordination and follow up on gaps.
• Utilize Epic registries to identify needs and action items.
• Outreach to patients and document attempts and outcomes.
• Schedule services and place referrals (vaccines, labs, mammograms).
• Follow up to track data and ensure continuity of care.
• Maintain 100% data accuracy and documentation.
• Communicate clearly with patients, care teams, and providers.
• Compile data for quality measures and projects.
• Attend staff meetings (approximately 80%).
• Identify barriers and coordinate resources with providers.
