Job summary

Carle Health in Champaign, IL seeks a qualified RN to join the Patient Clinical Services team. The role begins with six months of in-office training before transitioning to remote work, supporting primary care locations via telephone and electronic communications to triage and advise patients.

The RN will document patient health status, coordinate with physicians and nursing staff, ensure EMR accuracy, and provide education per approved protocols.

Qualifications

• RN licensure in Indiana (IPLA) within 1 year.
• RN licensure in Illinois (IDFPR) within 1 year.
• College Diploma: Nursing or equivalent.

Responsibilities

• Assist patients by phone and electronic platforms to determine care needs and schedule appointments.
• Document patient health status and care concerns.
• Coordinate with Primary Care physicians and nursing staff to meet patient needs.
• Maintain and ensure accuracy of electronic medical records, including medication reconciliation.
• Provide patient education and follow-up as per protocols.
• Direct patients to appropriate level of care and resources based on symptoms.
• Assist with inbound calls and triage; cross-train to float to other teams.
