Job summary

CVS Health in Phoenix, AZ is seeking a full-time Quality Practice Liaison for the Arizona Mercy Care Plan. The role is work-at-home, requiring presentation of HEDIS gap data to provider groups and abstracting medical records according to NCQA specifications.

You will collaborate with plan teams to improve member outcomes, access various EMR systems remotely, and meet volume and accuracy targets while reading both typed and handwritten documentation.

Qualifications

• Active and unrestricted Arizona RN license.
• 3+ years of clinical RN experience.
• 2+ years of experience with HEDIS quality measures.
• 1+ years of experience delivering presentations.
• Proficiency with Microsoft Outlook, Teams, Word, Excel, PowerPoint, Adobe Acrobat, Windows 10, and QuickBase.
• Willingness to travel in Arizona up to 10% of time, if commutable.

Responsibilities

• Generates and distributes gap in care reporting from Tableau Dashboard.
• Outreach to provider groups, scheduling meetings, and presenting HEDIS information.
• Review and abstract compliant medical record data for entry into the supplemental data tool.
• Contact providers, vendors, or internal departments to obtain additional information as needed.
• Demonstrate comprehensive knowledge of NCQA HEDIS and STARS measures for Medicare and Medicaid.
• Work with various electronic tools to achieve team goals.
• Performs other duties as needed.
