Job summary

CorVel Corporation is hiring a caring, self-motivated RN Field Case Manager to support injured workers in Milwaukee, WI. This role offers work-from-home flexibility with extensive local travel and regular business hours Monday–Friday.

You will collaborate with patients, families, medical providers, and the CorVel team to facilitate recovery, assess progress, and manage care plans. Travel up to ~60% and potential overnight travel are expected to meet client needs.

Qualifications

• Current RN Licensure in state of operation.
• CCM, CIRS, or other Case Management certifications preferred.
• Graduate of accredited school of nursing.

Responsibilities

• Provides in-person and telephonic Medical Case Management to patients, physicians, and providers.
• Discusses current treatment plans with physicians and discusses alternatives.
• Assesses, plans, implements, and evaluates patient progress.
• Evaluates treatment plan for medical necessity and cost efficiency.
• Attends doctors' visits, discharge planning conferences, and travels with patients.
• Conducts home visits for initial evaluation and coordinates care delivery.
• Regular local travel up to ~60% of work time; possible overnight travel.
• Performs additional duties as required.
