Job summary

Qualifications

• RN with current state licensure and nursing degree
• CCM/CIRS certifications preferred
• Willingness to travel locally and throughout defined jurisdiction
• Experience in case management or utilization review is helpful

Responsibilities

• Provide in-person and telephonic Medical Case Management to patients, families, providers, and employers
• Discuss treatment plans with physicians and discuss alternatives
• Assess, plan, implement, and evaluate patient progress
• Evaluate treatment plans for medical necessity and cost-effectiveness
• Attend doctors’ visits and discharge planning conferences as needed
• Conduct home visits for initial evaluation and ongoing management
• Coordinate delivery of durable medical equipment and nursing services
• Travel locally up to ~60% of time; overnight travel as needed
• Perform additional duties as required
