Job summary

Elevance Health is seeking a Nurse Disease Management I to support education and disease management across chronic conditions. You will provide health coaching, coordinate care plans with providers, and guide members toward appropriate services.

The role offers hybrid/remote work with in-person onboarding and training. Requires an AS in nursing, an RN license, and 2+ years in clinical/home health or discharge planning; BS in nursing is preferred.

Qualifications

• Requires AS in nursing and minimum of 2 years of condition specific clinical or home health/discharge planning experience.
• Current unrestricted RN license in applicable state(s) required.
• BS in nursing is preferred.

Responsibilities

• Performs health coaching and disease management interventions for members with chronic conditions.
• Conducts behavioral or clinical assessments to identify member needs.
• Coordinates health coaching plans with providers and networks.
• Educates members on clinical needs and facilitates referrals for behavioral health.
• Uses motivational interviewing to drive health behavior change.
• Monitors effectiveness of interventions and modifies plans as needed.
• Directs members to facilities and appropriate provider networks.
