Job summary

Novant Health is seeking an RN Coach for telephonic virtual care to help patients navigate their healthcare journey through chronic-care management, self-management support, and acute triage, collaborating with physicians and care teams to deliver safe, patient-centered care.

The role requires a BSN or higher (preferred), 5+ years of clinical experience, an active RN license in the NH footprint, strong teaching, coordination, and EMR documentation skills.

Qualifications

• BSN or higher degree preferred; graduate of an accredited nursing program required.
• Minimum five years of clinical experience in hospital, home health, or community settings with focus on acute and/or chronic-care management.
• Current RN license for NH footprint required; strong teaching, coordination, and EMR skills.

Responsibilities

• Provide telephone-based nursing support and triage using approved protocols.
• Support chronic-care management and self-management for patients.
• Assess acute healthcare needs and determine appropriate level of care.
• Connect patients with primary care, specialists, and community resources.
• Promote engagement, wellness, and effective navigation.
• Collaborate with interdisciplinary care team to improve outcomes.
• Deliver patient education and compassionate, holistic support.
• Adhere to privacy, compliance, and documentation standards in EMR.
