9amHealth is seeking a Registered Nurse to deliver patient-centered health coaching and chronic disease management via chat, phone, and video in a telehealth setting.

You will triage symptoms, educate patients on diabetes, hypertension, and weight management, and coordinate care with providers and cross-functional teams, while documenting care to internal standards. This role supports a remote-first work model with competitive benefits.

Qualifications

• BSN required and active RN license in good standing.
• Certificate: CDCES or willingness to pursue
• 3+ years direct patient care in ambulatory or telehealth
• Strong empathy, communication, and patient-centered care
• Proficient with digital tools for care delivery
• Able to work independently in telehealth setting
• Flexible for weekend or holiday coverage
• HIPAA compliant, distraction-free workspace

Responsibilities

• Deliver patient-centered health coaching via chat, phone, and video.
• Triage patient reported symptoms and escalate to Providers as needed.
• Provide care and education for diabetes, hypertension, hyperlipidemia, and weight management.
• Coordinate care with patients, providers, support teams, and cross-functional teams.
• Create consults that align with internal protocols for Providers.
• Document patient care accurately and consistently to meet internal standards.
