Qualifications

Responsibilities

• Conduct PCS and CAP assessments to determine LOC and eligibility for Medicaid-funded services.
• Ensure assessments are completed within timeframes and respect privacy and dignity.
• Coordinate with beneficiaries, families, and case management to develop approvable service plans.
