Job summary

INTEGRIS Health in Oklahoma City, OK is seeking an experienced RN Case Manager for Utilization Review to join our remote team. This full-time daytime role includes every-other-weekend shifts and focuses on coordinating care and discharge planning.

The ideal candidate has a BSN, an Oklahoma RN license (or compact) and Case Management Certification, with at least 2 years in a clinical setting. Strong communication and computer skills are essential, and a valid Oklahoma driver's license may be

Qualifications

2 years experience in a clinical settings (e.g. home health, inpatient, physician office, clinic).
RN licensure in Oklahoma or multi-state license via compact (eNLC).
Case Management Certification preferred.

Responsibilities

Completes comprehensive assessment of patient needs including medical, psychological and financial factors.
Coordinates transfers to appropriate levels of care based on clinical needs and utilization criteria.
Develops and revises discharge plans with referrals to healthcare and community resources.
Communicates the discharge plan and any changes to patient, family, and healthcare team.
Uses utilization criteria to optimize resource use and support timely discharge planning.
Facilitates patient transfers and collaborates with physicians to ensure appropriate care.
