Responsibilities

• Provide medical case management coordinating with patient, physician and care team.
• Assess, plan, implement, and evaluate patient progress.
• Evaluate treatment plans for medical necessity and cost effectiveness.
• Discuss treatment options with physician; coordinate durable medical equipment and nursing services.
• Prepare organized reports within required timeframes.
