Job summary

Duly Health and Care in Downers Grove, IL is seeking a RN, Case Manager - Nephrology for a full-time role. You will collaborate with physicians, patients, and families to promote quality outcomes for nephrology patients and manage a caseload across care settings.

The ideal candidate has an Illinois RN license, 2–4 years in primary care or hospital settings, and experience with case management. We offer a comprehensive benefits package and competitive compensation.

Qualifications

• Active RN license in the State of Illinois in good standing.
• Certification in Case Management preferred.
• Current CPR (BLS) certification must be maintained.
• 2-4 years of RN experience in primary care or hospital setting; case management preferred.

Responsibilities

• Collaborate with physicians, patients and families to promote quality outcomes for nephrology patients.
• Manage patients in multiple care settings to ensure appropriate level-of-care and prevent readmissions.
• Develop individualized care plans with prioritized goals and interventions for nephrology patients.
• Coordinate evidence-based treatment plans with multidisciplinary teams and educate patients and families.
