Job summary

CVS Health Corporation is seeking a full-time RN Case Manager to support members with Medicare/Medicaid, working from home with significant travel in Kane, LaSalle and surrounding counties in Illinois. You will assess needs, coordinate care, and advocate for appropriate benefits while promoting wellness and timely return to work.

The role requires an Associate’s Degree in Nursing (Bachelor’s preferred), an active RN license in Illinois, and 3+ years clinical experience.

Qualifications

• 3+ years clinical practical experience.
• Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually
• Excellent analytical and problem-solving skills
• Effective communication, organizational, and interpersonal skills
• Ability to work independently
• Proficiency with standard corporate software applications, including Microsoft Word, Excel, Outlook and PowerPoint
• Efficient and effective computer skills including navigating multiple systems and keyboarding
• 2+ years case management, discharge planning and/or home health care coordination experience
• Certified Case Manager

Responsibilities

• Coordinate and oversee case management activities across the continuum of care.
• Interact with members, providers, and caregivers to assess needs and plan care.
• Facilitate appropriate condition management and return-to-work planning.
• Prepare documentation and collaborate with multidisciplinary teams.
