Job summary

CVS Health, is seeking a Remote RN Case Manager to perform comprehensive LTSS assessments and develop person-centered plans for dual-eligible Medicare/Medicaid members. Travel 25–50% within Wayne and Macomb Counties for in-home visits, with a standard 8:00am–5:00pm schedule and no evenings or weekends after training.

Requires active Michigan RN license, 2+ years clinical nursing, 1+ year case management/home health, and proficiency with EMR systems. Bilingual skills preferred.

Qualifications

• Active, unrestricted RN license in Michigan.
• BSN or A.S. in Nursing preferred.
• Minimum 2 years clinical nursing experience.
• Minimum 1 year in case management/home health/long-term care.

Responsibilities

• Conduct in-home LTSS assessments to determine eligibility.
• Develop and implement person-centered care plans.
• Coordinate services across interdisciplinary teams.
• Educate members and caregivers about benefits and resources.
• Ensure compliance with Medicaid waiver requirements and CMS regulations.
