Job summary

CORE Health Networks is seeking a full-time Registered Nurse Case Manager working from home on a Monday–Friday schedule (8:30 am–5:00 pm CT). The role focuses on intake, care coordination, and timely updates to clients, providers, and adjusters throughout the injury management lifecycle.

You will triage calls, locate facilities, coordinate visits, and document all case activities in the Billing Portal. RN license in LA and CWCP within 2 years are required.

Qualifications

• Valid RN license for Louisiana or compact multistate license.
• CWCP certification must be obtained within 2 years.
• Minimum 2 years of nursing experience in a clinical setting.
• ER/ICU/PACU experience preferred.

Responsibilities

• Triage injury/illness calls to determine emergent needs.
• Coordinate treatment by locating facilities and arranging visits.
• Communicate with providers and adjusters regarding care plans.
• Document case details and billing entries in the system.
• Support 24/7 CM coverage and remote work as needed.
