Zusammenfassung

CVS Health is seeking a Remote RN Case Manager to support members across the U.S. from home. You will assess clinical, behavioral, and social needs by phone, create personalized care plans, and coordinate across providers to improve health outcomes.

Responsibilities include identifying barriers to care, reviewing eligibility and claims data, and engaging members with motivational interviewing. A fully remote, weekday schedule with potential on-call coverage is offered.

Qualifikationen

• Active, unrestricted RN license in state of residence.
• Ability to obtain multi-state/compact licensure (company-paid).
• 3+ years of clinical nursing experience.
• 1+ year of experience with Microsoft Office (Outlook, Word, Excel).
• Availability for Monday–Friday schedule with early start times and potential on-call.

Aufgaben

• Conduct telephonic assessments to evaluate members' medical, behavioral, and social needs.
• Develop individualized care plans to support improved health outcomes.
• Identify barriers to care and implement strategies to reduce risk and improve engagement.
• Coordinate care across providers, programs, and community resources.
• Review clinical information, claims history, and benefit eligibility to support interventions.
• Engage members using motivational interviewing and clinical judgment to support goals.
• Collaborate with multidisciplinary teams for complex cases and outcomes.
• Ensure compliance with regulatory requirements and company case management standards.
