The ideal candidate holds an RN license, has 5+ years clinical experience, and can work standard hours in their time zone, with a strong emphasis on HIPAA-compliant care and patient engagement.

Qualifikationen

• Active, current, unrestricted RN license in the state of residence.
• 5 years clinical practice as an RN.
• 2+ years’ experience in critical care.
• 6+ months case management or utilization management experience.
• Must work Mon-Fri 8 AM–5 PM in the residence time zone; no nights/weekends/holidays unless needed.]
• Must be able to obtain multi-state RN licensures.

Aufgaben

• Assess, plan, implement and coordinate all case management activities.
• Evaluate member needs and benefit plan eligibility.
• Facilitate transitions to Aetna programs and plans.
• Apply clinical judgment to reduce risk factors and barriers.
• Coordinate with supervisors and present cases at case conferences.
