You will collaborate with healthcare providers and community partners to improve outcomes, relying on strong analytical and communication skills, with familiarity in MS Office and secure systems.

Qualifications

• Must reside in Illinois
• Travel up to 75% around West Suburbs IL (DuPage, Kane, DeKalb, Kendall and surrounding areas)
• Minimum 3–5 years clinical practical experience
• Confident working from home and collaborating virtually
• Strong analytical and problem-solving skills
• Excellent communication, organizational and interpersonal skills
• Able to work independently
• Proficient in MS Word, Excel, Outlook and PowerPoint, plus proprietary apps

Responsibilities

• Coordinate case management activities and inform the process
• Assist with appointment scheduling and accessing benefits for members
• Educate members about benefits using care management tools
• Collaborate with members, providers, and community organizations to address health and social needs
