Job summary

Avardis Health Management is seeking a PRN RN Case Manager to lead patient care journeys from a home-based setting, with the flexibility of remote work. You will collaborate with a high-energy team to improve outcomes, access to care, and cost efficiency while delivering compassionate nursing leadership.

Responsibilities include shaping best practice initiatives, providing clinical leadership for Managed Care, Bundle Payment, and ACO programs, and developing comprehensive care plans with input

Qualifications

• Must possess a Degree in Nursing as a Registered Nurse.
• Must have at least two years' experience in a hospital, nursing care facility, or related health care facility.
• Working knowledge of managed care rules, regulations, and guidelines.
• Comprehensive knowledge of managed care, health insurance, and Case Management.
• Ability to negotiate coverage and provide complete and timely case management, bundled payment, and ACO reports.
• Prior experience with a Health Plan or third-party case management company preferred.
• Qualified, compassionate, and dedicated to a job well done.

Responsibilities

• Lead in shaping best practice initiatives to enhance health outcomes and access to care.
• Provide clinical leadership for Managed Care, Bundle Payment, and ACO programs.
• Develop comprehensive care plans with input from patients, families, and healthcare teams.
• Conduct proactive post-discharge follow-ups and physician consultations to remove barriers.
• Oversee data collection for quality indicators and drive improvements.
