WebTPA, a GuideWell Company, seeks an experienced RN Case Manager to create, implement, and monitor treatment plans for members. You will coordinate care with health professionals, assess needs, and ensure evidence-based, cost-conscious plans.

Requirements include 5+ years in clinical nursing with a related bachelor’s or equivalent, RN licensure, and strong collaboration skills to work across departments and with employers/clients.

Qualifikationen

• 5+ years related work experience with a professional background in clinical nursing and patient assessment.
• Bachelor’s degree or equivalent work experience in a related field.
• Graduate of an accredited school of nursing and RN licensure.

Aufgaben

• Assess member health status and care coordination needs, including discharge planning and outpatient management.
• Coordinate with patient, family, physicians, and providers; communicate with employer/client when required.
• Identify and advocate for alternative treatment plans within cost-containment guidelines.
• Monitor health status and adjust care plans as client needs evolve.
• Identify services and resources to promote value and timely care.
• Develop and implement short- and long-term case management goals with documented plans.
• Evaluate cost-effectiveness using available data and report outcomes to clients.
