Job summary

WebTPA, a GuideWell Company, is seeking an experienced RN Case Manager to create, implement, and monitor treatment plans for members, coordinating care across providers and services to meet healthcare goals.

This role emphasizes assessment, collaboration with families and providers, and cost-conscious care planning within managed care environments. Hours are Mon-Fri 8am-5pm Central Time; remote-friendly options may apply.

Qualifications

• 5+ years related work experience with a professional background in clinical nursing and patient assessment.
• Graduate of an accredited school of nursing and RN licensure.
• Knowledgeable in medical terminology, reasonable and necessary treatment plans.

Responsibilities

• Assess member health status and care coordination needs, including discharge planning.
• Coordinate with patients, families, physicians, and providers to develop appropriate care plans.
• Evaluate clinical status and adjust care plans to meet changing needs.
