Qualifications

• Case management and discharge planning experience preferred.
• 2 years experience in behavioral health or related field equivalent to program focus.
• Managed Care experience preferred.
• Ability to travel within a designated geographic area for in-person case management activities.

Responsibilities

• Evaluate member needs and eligibility and recommend approaches to case resolution.
• Identify high risk factors and coordinate appropriate referrals to care teams.
• Coordinate and monitor care plan activities and progress.
• Collaborate with care teams to ensure optimal outcomes and document care.
