Ideal candidates hold an RN license in NC or a compact state, with 3 years of clinical/case management experience, and must obtain CCM certification within 2 years of employment. Hybrid flex options available across the region.

Qualifications

• RN with 3 years of clinical and/or case management experience.
• Active RN license valid in NC or compact multi-state.
• CCM certification to be obtained within 2 years of employment.

Responsibilities

• Coordinate care, resources and services for members to achieve outcomes.
• Perform comprehensive assessments and develop member-centric plans.
• Monitor progress, adjust plans, and document all actions.
• Educate members and promote proactive care while managing costs.
