Qualifications

• Active, unrestricted RN license in MA
• Bachelor’s degree or equivalent required; bachelor’s degree to be completed within 3–5 years per role development
• Certified Case Manager preferred; national certifications encouraged
• Willing to obtain multi-state RN licenses if needed; travel may be required

Responsibilities

• Assess and plan individual and family health needs using collaborative processes
• Coordinate discharge planning and home health care services
• Advocate for options and resources to meet quality and cost-effective outcomes
• Travel within designated geographic area for in-person case management as directed by leadership
