The role requires NYS RN licensure, BSN preferred, and at least one year of nursing experience. Strong communication and computer skills are essential, with potential loan repayment eligibility.

Qualifications

• RN license active in New York State.
• Minimum of 1 year of nursing experience.
• Knowledge of home care, Medicare, and Medicaid is a plus.

Responsibilities

• Provide care management for a caseload through assessment, planning, and advocacy.
• Coordinate services and document care per health plan standards.
• Oversee care management team activities and ensure timely, compliant documentation.
