Job summary

Molina Healthcare is seeking a remote Care Manager RN to support Medicaid members transitioning from incarceration in Washington. The role focuses on disease management education and identifying eligible resources to help members re-enter the community.

The candidate must hold an active multi-state RN license, have at least 2 years in healthcare or care management, and be able to work Pacific Time hours (8 AM–5 PM, Mon–Fri). Travel up to 25–40% may be required depending on state commitments.

Qualifications

• Active RN license in good standing (multi-state/compact).
• Minimum 2 years experience in health care, preferably care management or related settings.
• Must be willing to work Pacific Time hours (8 AM–5 PM, M–F).
• Knowledge of EMR systems and HIPAA regulations.

Responsibilities

• Complete comprehensive member assessments within required timelines.
• Develop and implement care coordination plans with the member and care team.
• Conduct telephonic, in-person, or home visits as needed.
• Monitor and adjust care plans; document interventions and outcomes.
• Maintain an ongoing member caseload with proactive outreach.
• Promote integrated care across behavioral health, LTSS and community resources.
• Facilitate interdisciplinary care team meetings and collaboration.
• Use motivational interviewing techniques to educate and support change.
• Identify barriers to care and provide coordination assistance.
• May provide consultation to peers and participate in medication reconciliation.
