Centene is seeking a Nurse Care Manager to develop and facilitate complex care management for primarily physical needs members within Missouri. The role supports education for members and families, with a remote work option and a 8am-5pm weekday schedule.

Qualifications
- Requires an accredited nursing degree (RN) and 2–4 years of related care management experience.
- RN license in the state of Missouri or compact state license.
- Experience coordinating care plans and collaborating with providers.
Responsibilities
- Assess member needs and barriers to care, focusing on prioritized health goals.
- Develop care/service plans and coordinate with providers and community resources.
- Provide ongoing follow-up and monitor changes in member status to update plans.
- Facilitate timely access to needed care and services through care team collaboration.
- Educate members and families on disease processes and care options.
- Maintain required member documentation in compliance with state and payer regulations.
- Support leadership by identifying opportunities to improve care delivery.
