Job summary

CircleLink Health is hiring a registered nurse to work remotely as an RN Care Coach in Medicare Chronic Care Management. This part-time role (20-25 hrs/week) will involve following a group of patients monthly, offering education, coordinating care, and coaching for self-management to prevent hospitalizations.

You will use specialized care management software to reach patients with chronic conditions, build rapport, and update care plans while coordinating with community resources.

Qualifications

• Fluent in English.
• Self-directed, able to work independently with little supervision while meeting performance metrics.
• Passion for nursing and improving patient outcomes.
• Good with technology and eager to learn and use new software.
• Excellent organizational and time management skills.
• Strong communication and telephonic skills.
• Strong critical thinking and problem solving skills.

Responsibilities

• Utilize care management software to call Medicare patients with 2+ chronic conditions on a monthly basis.
• Build and maintain rapport to coach patients toward SMART health goals and education on self-management strategies.
• Update Plan of Care by tracking medications, appointments, biometrics, symptoms, and interventions.
• Connect patients with community resources (transportation, personal care, prescription/DME, social services).
• Conduct Transitional Care Management for high risk patients discharged from hospital/ER to reduce readmissions.
• Close care gaps by promoting preventive care measures.
